Client Intake

ExpertMD provides attorneys with a client intake form system. You can create a branded intake form with a custom URL, share it with your clients, and receive submissions directly in your ExpertMD dashboard. This streamlines the process of gathering case information from clients before engaging a medical expert.

Overview

The client intake system allows your clients (plaintiffs, defendants, or other parties) to submit case information to you through a dedicated form. This information helps you prepare intake requests for medical experts on the platform.
Client intake is separate from the intake requests you submit to doctors. Think of it as: your clients submit information to you through your client intake form, and then you submit intake requests to doctors based on that information.

Setting up your intake form

Configuring your URL slug

Every attorney on ExpertMD gets a unique client intake URL. You can customize the slug to make it professional and easy to remember.

Navigate to Settings

Click Settings in the left sidebar, then select the Intake tab.

Set your URL slug

Enter your desired URL slug. Your intake form will be accessible at:
https://expertmd.io/client-intake/your-slug-here
Your slug must:
  • Be unique across all ExpertMD users
  • Contain only lowercase letters, numbers, and hyphens
  • Be between 3 and 50 characters

Save your settings

Click Save to activate your intake URL. You can change your slug at any time, but the old URL will stop working immediately.
Choose a slug that is professional and easy to communicate verbally. Good examples: smith-law, johnson-associates, miami-injury-law. Avoid overly long or complex slugs that are hard to type.

Intake form fields

Your client intake form collects the following information from clients:

All submitted information is stored securely. Sensitive data within the submission is protected by ExpertMD’s end-to-end encryption once processed into a case.

Sharing your intake form

Once your intake URL is configured, share it with clients through multiple channels: Copy your intake URL from Settings > Intake and share it via:
  • Email — Include the link in emails to new or existing clients
  • Text message — Send the link via SMS for quick mobile access
  • In-person — Share the URL during client consultations

Website integration

Add your intake link to your law firm’s website:
  • Add a “Submit Your Case” button that links to your ExpertMD intake URL
  • Include it on your Contact Us page
  • Add it to your website’s navigation menu

Email signature

Include your intake URL in your email signature so every email you send gives clients easy access to submit their case information.
When sharing your intake URL with clients, briefly explain what it is: “Please use this secure form to submit your case details. The information is encrypted and will help us find the right medical expert for your case.”

What clients see

When a client visits your intake URL, they see a clean, professional form with:
  • Your name and firm name at the top
  • Clear field labels and descriptions
  • File upload capability for supporting documents
  • A submit button that confirms their submission
The form is mobile-friendly, so clients can complete it from their phone or tablet.

Receiving submissions

When a client submits the intake form:
  1. You receive a notification in your ExpertMD dashboard
  2. You receive an email alert (if email notifications are enabled)
  3. The submission appears in your Intake Requests section

Reviewing submissions

Navigate to your intake requests page to see all client submissions. Each submission shows:
  • Client name and contact information
  • Case type and description
  • Date submitted
  • Attached documents
  • Status (New, Reviewed, Converted to Case)

Converting to a case

After reviewing a client submission, you can use the information to:
  1. Search for an appropriate medical expert on ExpertMD
  2. Submit an intake request to the doctor using the client’s case details
  3. Once the doctor accepts, a case is created that you can manage through the platform

Managing your intake form

Enabling and disabling

You can temporarily disable your client intake form from Settings > Intake:
  • Active — Your form is accepting new submissions
  • Paused — Your form shows a “Not accepting submissions” message
This is useful if you are at capacity and do not want to receive new client inquiries.

Updating your slug

You can change your URL slug at any time from Settings > Intake. Keep in mind:
  • The old URL will stop working immediately
  • Update any links on your website, email signature, or other materials
  • Existing submissions are not affected
If you change your slug, anyone using the old URL will see a 404 page. Make sure to update all references to your intake URL when you change it.

Troubleshooting

My intake form is not accessible

  • Verify your URL slug is saved correctly in Settings > Intake
  • Check that your form status is set to Active
  • Try accessing the URL in an incognito browser window
  • Ensure you are using the correct URL format: https://expertmd.io/client-intake/your-slug

Client cannot submit the form

  • Ensure all required fields are filled in
  • File uploads must be under the maximum size limit (typically 10MB per file)
  • The client’s browser must support modern web standards
  • Disable browser extensions that may block form submissions

I am not receiving notifications for submissions

  • Check Settings > Notifications to ensure intake notifications are enabled
  • Verify your email address is correct in your profile
  • Check your email spam folder
